WELCOME TO THE CJD
MEMORY QUILT
The Memory Quilt is an
opportunity for people who
wish to make contributions
to honor or memorialize a
loved one or a friend. For a
donation you will be able to
establish a "square" on the
CJD Memory Quilt. In this
square you may place either
a photograph or a star along
with a tribute of 10-100
words.
CREATE A TRIBUTE
A "square" on the CJD Memory
Quilt will be created with
your donation of $100.
Please complete the
following form. When you
click the "submit" button,
you will be taken to a
special donations page where
you can provide the
requested information. Your
Memory Quilt entry will be
sent to CJD Foundation's
webmaster for approval and
posting. You will be
notified by email when your
memorial has been added.
ADD A TRIBUTE
A donation of $50 or more
will enable you to add your
own tribute to any existing
tribute square. The tribute
can be 10-100 words. To
view existing tributes,
click on the photograph or
star on the Memory Quilt.
ACKNOWLEDGMENTS
When a CJD Memory Quilt
"square" is created or a
tribute is added to a square
on the quilt, a thank you
will be sent to the person
making the donation. An
acknowledgment will also be
sent to the family member
listed on the donation form.
Please be sure to use the
CJD MEMORY QUILT Donation
form to add to the CJD
Memory Quilt.
TIME FRAME FOR
POSTINGS AND TRIBUTES
Tributes will be posted on
the website on the 1st or
16th of each month. They
will remain posted for one
year. Any additional
tributes will be posted
along side the original.
They will remain posted as
long as the original posting
is present.
